Family Nurse Practitioner Needed in Phoenix, AZ
Start Date: As soon as credentialed
Credentialing timeframe: Standard clinic credentialing
Contract Length: Ongoing contract (Locum Tenens – 1099)

Schedule

Full‑time outpatient schedule
Monday – Friday
Day shifts: 9:00 AM – 5:00 PM
No call
No weekends


Practice Details

Outpatient primary care clinic
Integrated care team environment
Support staff includes:

Medical Assistants
Registered Nurses
Care coordinators
Behavioral health staff
Front desk support


Patient population: All ages
Practice setting: Clinic / private practice


Clinical Scope

Comprehensive family medicine and primary care
Responsibilities include:

Preventive care and health maintenance
Diagnosis and management of acute and chronic conditions
Follow‑up care and patient education
Documentation of all patient encounters
Participation in a patient‑centered care team
Oversight of Physician Assistants as applicable




Requirements

Nurse Practitioner – Family Practice
Board Certified in Family Medicine
Active Arizona NP license required (IMLC accepted)
DEA required
BLS required
ACLS preferred
Minimum 3 years outpatient family practice experience preferred
Community health or comparable outpatient experience preferred

Clean credentialing file strongly preferred

Credentialing

Clinic credentialing required
Provider must be fully cleared prior to start
