Assignment Details
• Start Date: ASAP
• Contract Length: Ongoing
• Schedule: Flexible Scheduling; Monday – Sunday (no nights/weekends required)
• Practice Setting: Home Health
• Case Mix: 6–9 in-home evaluations per day; Conduct scheduled in-home health assessments for Medicare, Medicaid, and commercial plan members, including medical history review, medication review, and physical evaluations. No prescribing, lab orders, or treatment changes required.

Requirements
• State License: Active OH License Required
• Certifications: Must be Board Certified; Master’s Degree Required
