Schedule
-Flexible scheduling available Monday through Sunday
-Day shifts only: 7:00 AM – 7:00 PM
-No nights or weekends requirement

Practice Setting
-Home Health
-In-home patient evaluations

Position Details
-Providers will conduct comprehensive in-home health assessments that include: Medication review, Medical history review, Physical evaluation
-All visits are pre-scheduled, and providers are supplied with a pre-populated, patient-specific form for documentation. This role does not include prescribing medications, ordering labs, or modifying treatment plans.
-Average Visits Per Day: 6–9
-EMR: TBD

Requirements
-Board Certification Required
-Active State License Required
-Master’s Degree Required

Credentialing Timeline
-Fast credentialing available
-Estimated timeframe: 7 days to 2–3 weeks
