🩺 Medical Director – Primary Care / PACE (Permanent)
📍 Location: Philadelphia Area, Pennsylvania
⏰ Schedule: Full-Time | Monday–Friday

⚡ THE HIGHLIGHTS:
Lead the medical direction of 11 community-based PACE programs.
Blend of administrative leadership and direct patient care.
Supervise Primary Care Physicians and Nurse Case Managers.
Oversee the Quality Assurance Performance Improvement (QAPI) program.
Travel to program sites, nursing homes, and participant care centers throughout the Philadelphia region.

📋 CORE RESPONSIBILITIES:
Provide medical leadership across multiple PACE locations.
Supervise Primary Care Providers (PCPs) and Nurse Case Managers.
Develop and maintain medical policies, procedures, and clinical protocols.
Lead the Quality Assurance Performance Improvement (QAPI) program.
Conduct participant assessments and develop individualized treatment plans.
Coordinate care across outpatient, nursing home, and community settings.
Lead interdisciplinary team meetings and provide clinical guidance.
Participate in advance care planning and discussions with participants and caregivers.
Manage emergency medical situations when necessary.
Ensure accurate EMR documentation and compliance.
Collaborate with specialists, consultants, pharmacists, and healthcare teams to optimize patient outcomes.
🛑 SUBMISSION REQUIREMENTS (Strict Compliance):
Active Pennsylvania Physician License – Required.
Board Certified in Internal Medicine or Family Practice – Required.
Board Certification in Geriatrics – Highly Preferred.
DEA Registration – Required.
CDS Certification – Required.
BLS Certification – Required.
Minimum 1 year of experience caring for frail or elderly patients – Required.
Previous physician supervisory experience – Required.
Strong chronic disease management experience for geriatric populations – Required.
Ability to work in a community-based healthcare setting – Required.
NPDB Self-Query Report (within 30 days) – Required at presentation.

☎️ CALL SCHEDULE:
Call Required: Yes
Participate in the provider on-call rotation.
Typically two one-week call assignments, with approximately one call every 3–4 months.
💰 COMPENSATION & BENEFITS:
Salary: $250,000–$290,000 annually
