Job description

• Provider needs to be open to working on a hybrid team (CAA and CRNA)
• Cross credential: NA
• Credentialing timeline: 90 days
• Start: ASAP
• Shift: 3x12s alone or with an additional 8, 10 or 12 shift OR 2x12s + 2x10s or 2x8s (need nonconsecutive)
• No T&L for anyone within a 60-mile radius (proof of residency required). Outside of 60 miles, we offer an all-inclusive rate.
• Cases: General, ortho, blocks, OB
• EMR: Medaxion
Submission Requirements
• MUST fill Submission Questionnaire and attach to presentation file
• MUST double check and report malpractice history
• Resume
• Schedule request and call availability (if applicable)
• Available weeks
• RTO
• Tentative start date
• Travel/expenses (if applicable)
Certification Requirements
• ACLS
• BLS
• PALS
State License Requirements
• Texas
Additional License Requirements
• 2 years’ experience. Clean file.
