Job Summary
Helen’s Project is seeking a detail-oriented and community-focused Case Manager / Discharge Planner to support clients in coordinating care and accessing appropriate community resources following treatment.
This role plays a critical part in ensuring continuity of care by assisting clients with discharge planning, connecting them with local services, and conducting follow-up care check-ins.
The ideal candidate is organized, compassionate, and able to collaborate effectively with healthcare providers, clients, and community organizations.
Duties and Responsibilities
Coordinate discharge planning and ensure continuity of care for clients
Conduct 30-day follow-up care check-ins to ensure clients are successfully connected to recommended services
Connect and collaborate with community resources and support programs
Serve as a liaison between clients, healthcare providers, and community partners
Assist clients in navigating available social and healthcare services
Maintain accurate and timely documentation of client interactions and care coordination
Communicate effectively with interdisciplinary teams to support positive client outcomes
Requirements and Qualifications
Bachelor’s degree required (Social Work, Psychology, Public Health, Healthcare Administration, or related field preferred but not required)
No professional license required
Strong organizational and documentation skills
Detail-oriented with the ability to manage multiple cases
Community-focused with a passion for connecting individuals to supportive services
Excellent communication and collaboration skills
Ability to work independently in a remote work environment
Compensation
