Harbor of Hope LLC is a licensed Substance Use Disorder and Mental Health treatment facility located in Portland, Oregon. We are an OHA-certified ASAM Level 2.5 program contracted with Trillium Community Health Plan providing peer support, residential, and outpatient SUD and mental health treatment services to the Portland community.
We are seeking a Licensed Clinical Social Worker (LCSW) to join our team as a full or part-time employee. This is a W-2 employee position with competitive pay and benefits.
Responsibilities:

Conduct comprehensive biopsychosocial assessments for new clients
Develop and monitor individualized treatment plans for SUD and co-occurring mental health conditions
Provide individual and group therapy sessions using evidence-based approaches
Deliver trauma-informed, person-centered care to clients in residential and outpatient settings
Coordinate care with Medical Director, peer support staff, and case managers
Maintain accurate and timely clinical documentation in CollaborateMD EMR
Participate in interdisciplinary treatment team meetings
Assist with discharge planning and community resource referrals
Ensure compliance with OHA regulations and Trillium billing requirements
Support clients in navigating Oregon Health Plan and community services
Provide crisis intervention and risk assessment as needed
Collaborate with families and support systems as appropriate

Schedule:

Full-time or part-time hours available
Monday through Friday
Mix of on-site and telehealth sessions available

Requirements:

Active unrestricted Oregon LCSW license — required
Experience working with SUD and co-occurring mental health populations — required
Knowledge of ASAM criteria and level of care assessments preferred
Familiarity with OHA Medicaid and Oregon Health Plan billing preferred
Experience with trauma-informed care models
CADC certification a plus but not required
Strong documentation and clinical writing skills
Ability to work independently and as part of an interdisciplinary team
