Gastroenterology locum in Midland, MI-Jan 16th start-MI license required-24 Hr Call-7a to 7a-Dates in post-$3,500 daily & $375 Hr callback

We have an urgent need for the following dates:
Orientation would need to be 1/16.
1/19-1/26
2/2-2/9
2/16-2/23
3/2-3/9
3/16-3/23

Call only (7am - 7am)

Schedule:
One day of Orientation, prior to first date of coverage

Orientation rate: flat rate of $500.00 for 4 hours or less of orientation. For orientations that go longer than 4 hours, the flat rate will be $1,000.

Coverage dates needed:
1/19 @ 7a – 1/26 @ 7a
2/2 @ 7a – 2/9 @ 7a
2/16 @ 7a – 2/23 @ 7a
3/2 @ 7a – 3/9 @ 7a
3/16 @ 7a - 3/23 @7a

Case Logs (Required Previous Experience):

All applicants must be able to demonstrate that he or she has provided inpatient or consultative services for at least 24 patients in the past 12 months and documented in that time he or she has had the following minimal endoscopic experience within the past 12 months:

A. Diagnostic EGD: 100 procedures

B. Total Colonoscopy: 100 Procedures

C. Snare polypectomy: 20 Procedures

D. Nonvariceal hemostasis (upper and lower): 10 active bleeders and total of 20 procedures

E. Variceal hemostasis: 5 active bleeders and a total of 15 procedures

F. Esophageal dilation with guide wire: 15 procedures

G. Flexible sigmoidoscopy: 25 procedures

H. PEG: 10 procedures

I. Diagnostic ERCP: 75 Procedures

J. Therapeutic ERCP: 25 procedures (includes 20 sphincterotomies and 5 stent placements)

K. Tumor ablation: 20 procedures

L. Pneumatic dilation for achalasia: 5 procedures

M. Esophageal stent placement:

EMR and Dictation Tools:
EPIC/Dragon

Required experience, training, and certifications:
BC/BE
BLS

Consider IMLC or waiting for a license:
MI license only due to timeframe

Credentialing Timeframe:
Typically 30 days, but applications are prioritized according to start date
