Both Clinic & Hospital privileges needed
Rounding & Admissions, Yes discharges are required as well

Avg. patients per day 12-14

Required Procedures- N/A
Clinical Activity past 24months- Applicant must provide documentation (case list) of provision of clinical services (100 patients) representative of the scope and complexity of the privileges requested during the previous 24 months. Case list must include diagnosis or procedure and date, and facility where performed.
Provider will be working Solo- Yes
